If I was starting from scatch without an existing towbar, I'd have a purpose-built bike rack made, bolting directly to each chassis rail - to distribute the load over the full width of the vehicle chassis - much more stable than suspending a bike and rack from an existing towball with the weight three feet either side of the only support.
I would make the wheel channel hinged near one end, and the other end lowers to the floor, push the bike on, and using a bottle jack, lift the lower end and lock it in place, that way you don't need to carry a seperate ramp.
You can have it made so your bike is as close the the back of your van as possible. The one pictured above stands 24 inches out from the towball, the one I had made only sticks out six inches from my towball (which it doesn't use by the way), and 11 inches out from the rear of my van.
Mine's not as outlined above, I had made two slot-over wheel supports which fit over my Hope Safe-T-bar, together they only weigh 12 kilos
