Well, I took the plunge and got myself a 15" Macbook Pro, and a 24" monitor. Both screens are glossy which gives the good contrast I need and reflections haven't been too much of a problem so far.
I am now finding my way around and transferring stuff from my PC. What really works for me is the monitor, plus following the advice you gave on here. As I get to know the systems and menus I'm finding using the Macbook on its own easier to use for short periods.
I'm glad I bought a new Macbook, rather than a reconditioned one as John did. This has meant that I was able to get the One to One package which means I can have 1-1 or group tutorial sessions, for a year, to learn all aspects of using my computer, iPhone etc. The support has been fantastic.
This lets even those with poor sight hearing or manual dexterity use a mac
They even have voice control and voicing of all the menu options
Note that there is some criticism of the graphics in 10.5 compared with 10.4

Yes, I've used this to enhance contrast, changing to white on black, and for zooming. I zoom quite a bit, though it does make me a bit 'seasick'.
I still think what would really, really help is if Apple would make their graphics generally more visible, including on the keyboard. I'd then be able to see easily what key to press to enhance my vision on the screen.
