Id say there was more to it than just a bulb gone, if they are all flashing once the brake is applied. Im guessing theres some terminals touching in the light unit - possibly initially blowing said bulb.
yes aggreed...but just have in mind the light cluster has a common earth and thats also worth looking at, could be rusted up... but after replaceing the faulty lamp..... hope that helps

Maybe EBA (Emergency Brake Assist) is activating as that forces harzard on rapid flash?
No idea if your van has them mind, nor no idea if its related. But maybe!

I go with Clive in saying it is an earthing fault, so once bulbs and their connections in the holder have been checked- sometimes the bulbs get a build up of muck between them and the holder which breaks the contact. If that dosn't cure the problem start looking at the connections between the lamp and the chassis.
Definitely an earthing fault, get someone to check and you will probably find your brake lights are also flashing dim/bright with the indicators.
